Passports
Scanning of the main page of the passport details
Scanning of the page of the passport cover
Scanning of Kitas Stamp on the passport
Copy Of Original Kitas with legalize Stamp by Immigration office that issued Kitas
Marriage / Divorce Certificate (in English / Indonesia) – if Any
Birth Certificate (in English / Indonesia)
Formal Photo (with red backround)
Address in Bali (must be same with kitas address)
Valid Domicile Letter From Banjar/Neighborhood head
Domicile Letter Approved by Urban Village & subdistrict (if Any)
KTP (ID Card) & Kartu Keluarga (Family Certificate) Sponsor from same district
